This form will not be submitted to the NCCSA until (1) you click the "Submit" button at the bottom of the form and (2) you receive the confirmation email which will be sent to the email address submitted on the form.

If you do not receive the confirmation email within 30 minutes, check your junk mail folder. If you do not receive the confirmation email after checking your junk mail folder, your form was likely not submitted and you will need to contact the NCCSA State Office for further assistance.

  • Schools must submit the required information each school year (by the deadlines listed in the policy) for each 7–12th grade homeschool student who wishes to participate in NCCSA Athletics or Fine Arts.
